Academic Integrity​​

FACULTY SHOULD COMMUNICATE WITH STUDENTS REGARDING ACADEMIC MISCONDUCT MATTERS BEFORE REFERRING THE MATTER TO STUDENT CONDUCT.

Use this form to report incident of academic misconduct. Examples include, but are not limited to: cheating, plagiarism, fabrication or falsification of data, or use of paid or unpaid services to complete an assignment.

THIS REPORT IS FOR NON-CRITICAL OR NON-URGENT REPORTING OF CONCERNING STUDENT BEHAVIOR.

A CARE Report is provided to encourage faculty, staff, students and other community members to report observed behavior that warrants concern for the safety of the campus and the individuals involved. Members of the Care Team can provide support to students through referral follow-up. If the behavior is disruptive, please submit a Student Discipline Report.
